Willow Bend Family Medicine, P.A. participates with many insurance plans. Below is a list of these plans. Each carrier has multiple products, we are not participating in all plans. For more information or to learn if we are in-network with a plan that is not listed, please call our office at (972) 981-7000.
The Health Market Exchange Plans/ACA (Accountable Care Act)/Healthcare Reform Act, please call our office for accurate participating information.
We request that patients present their health identification cards at the time of service for every visit. This is necessary for our office to ensure correct insurance billing and payment information and to prevent fraud. If this information is not available, the patient (guarantor) will be responsible for payment in full at the time medical services are rendered.
We will bill all insurance plans that we are participating with. Remember that your coverage is a contract between you and your insurance company. It is your responsibility to understand the basics of your coverage. You are liable for any amount not contractually covered by your insurance plan. It is not possible for the office to know whether or not your policy will cover a particular problem or service, so please call your insurance company and review any questions you may have regarding your explanation of benefits with your insurance representative. No insurance payer will guarantee payment until your claim is received and processed. Please call the office at (972) 981-7000 for in-network participation on the plans listed below especially the ones marked with (*) for specific information.
We will see patients who do not have health insurance. Payment in full is required at the time medical services are rendered. The cost of medical care is determined by a variety of factors, including but not limited to the time required to deal with your illness/injury, the number of problems, the complexity and chronicity of your illness/injury, the cost of materials and equipment, diagnostic tests ordered, and the risk of your illness/injury and of the medications used to treat your illness/injury.
Payment Information
All insurance co-payments, deductibles and co-insurance are expected at the time of service. We accept cash, MasterCard, Visa, American Express and Discover. Unless prior payment arrangements have been made with the business office all outstanding balances are due prior to receiving medical services. Payment for services not covered by your health insurance are payable at the time, i.e. supplies; ankle boots, arm slings, ace bandages and some other items. Patient invoices may be paid online through our secure patient portal.
Returned Check
Personal checks are accepted only when mailing in an account payment. The return by any financial institution of a dishonored payment issued to Willow Bend Family Medicine, P.A. will result in a service fee of $30.00 per dishonored check. Two dishonored checks will require that all future payments be made with cashier’s check, money order, credit card or cash.

Texas Health Presbyterian Hospital of Plano
Dr. William A. Calley Jr., admits directly to the facility and provides patient hospital services. Our Doctors are Board Certified Family Physicians that utilize the Hospitalist program at Texas Health Presbyterian Hospital Plano.
