Patient Forms
We request you print our Patient Forms and bring them with you for your scheduled appointment. Please arrive 15 minutes early for your appointment with patient forms completed, your health insurance information and your drivers license available for review and copying.In order to print them, you'll need Adobe® Acrobat Reader (version 5.0 or later). You may download the program for free by clicking here, A new window will give you the download information for the reader from the Adobe website.

If you are coming in for a Physical:
- Multiple issues during a physical may require a separate appointment.
- Do not have anything to eat or drink after midnight the night before your appointment. You are
encouraged to drink plenty of water and take your medication(s). You may also consume one cup
of black coffee or tea, but with no cream or sugar.
- Do not apply lotions or oils to your skin the morning of your physical. This prevents the EKG leads
from adhering to the skin.
- Bring all your prescription medications with you even if they were not prescribed by one of our
providers
- Please allow 1 to 2 hours for you physical appointments.
- Please contact your insurance company to find out the specifics of your benefits and ask about
preventive services to avoid any non-covered services. We will collect your co-payment, deductible, coinsurance at the time of medical service and file the claim with your insurance company. Any services not covered by your insurance company will be your responsibility.
- There is a $75.00 fee if you miss your scheduled physical appointment. Please call at least 24 hours in
advance to cancel an appointment.
